If you or your child need orthodontic treatment, you've likely hit the same question: braces or aligners? Both straighten teeth, but they work differently, cost differently, and suit different situations. Here's a clear guide for patients in Karachi.
How each system works
Traditional braces use metal (or ceramic) brackets bonded to each tooth, connected by a wire. Your dentist tightens the wire at each visit — progressively pulling teeth into position. Treatment is continuous and involuntary: you can't remove them.
Clear aligners (like Invisalign or locally sourced alternatives) are a series of custom-made transparent trays. You wear each tray for about two weeks, then switch to the next. Each tray moves teeth a fraction of a millimetre. You remove them to eat and clean your teeth.

Aligners cost more upfront in most cases, but the right choice isn't always the cheapest one.
Which straightens better?
For most cases, both deliver comparable results. But there are exceptions:
Braces are generally better for:
- Severe crowding or complex tooth movements
- Large gaps requiring significant movement
- Children and teenagers who may not comply with wearing aligners 20–22 hours a day
- Cases requiring precise control over each tooth's rotation and root position
Aligners tend to be better for:
- Mild to moderate crowding and spacing
- Adults who want discretion at work or in social settings
- Patients with good discipline (wearing aligners less than prescribed significantly slows or stops progress)
- People who play contact sports or wind instruments
Treatment duration
Mild cases: 6–12 months with either system.
Moderate cases: 12–18 months typically.
Complex cases: 18–30 months — braces often finish faster here because the dentist has more control.
Aligners don't necessarily move teeth faster, despite what some marketing suggests.
The compliance factor
This is the most underappreciated point in the braces vs aligners debate. Aligners only work if you wear them. The recommended minimum is 20–22 hours per day — they come out to eat and to brush, and that's it.
For teenagers especially, this is a serious consideration. Many parents choose braces specifically because compliance isn't optional.
Hygiene
Both require extra care, but differently:
- Braces: food traps around brackets; requires meticulous brushing and flossing around wires. Certain foods (hard, sticky, crunchy) must be avoided entirely.
- Aligners: you remove them to eat anything you like, but trays must be cleaned after every meal before reinsertion. Drinking anything other than water with them in is not recommended.
Retainers — the often-skipped step
With either system, you'll need a retainer after treatment. Teeth have a memory and will drift back without it. At our clinic, the retainer is included in the braces package. This is worth checking with any clinic you consider.
Who should I see?
Look for a PMDC-registered dentist with specific orthodontic experience and a record of completed cases. Ask to see actual patient photos — before and after — not stock images. A good clinician will assess your specific bite and tooth positions before recommending a system.
Frequently asked questions
At what age should my child get braces?
Most orthodontists prefer to wait until most permanent teeth have erupted — typically 11–14 years. An early assessment at 9–10 helps plan timing.
Can adults get braces?
Absolutely. There's no upper age limit. Adult treatment may take slightly longer because jaw bone is denser, but results are the same.
Does it hurt?
Both systems cause some tenderness after each adjustment or tray change — typically 2–4 days of mild soreness. This is manageable with over-the-counter pain relief.
Can I switch from braces to aligners mid-treatment?
In some cases yes, but it's better to commit to one system from the start. Switching adds cost and resets some of your timeline.
