Scaling & Polishing: Why You Need It Every 6 Months (And What Skipping It Costs You)

Of all the treatments in dentistry, scaling and polishing has the highest return on investment. It costs PKR 5,000–7,000 and takes 45 minutes. Done twice a year, it prevents problems that cost PKR 20,000 to PKR 350,000 to fix. Yet most patients in Karachi only come in for it once they can feel the tartar — or once they've already been told they have gum disease.

Here's what's actually happening in your mouth between appointments, and why twice a year isn't being cautious — it's the correct clinical interval.

What scaling and polishing actually removes

Your mouth contains hundreds of species of bacteria. Most are harmless. The problem is the biofilm — a structured colony of bacteria — that forms on every tooth surface within hours of cleaning. This is plaque.

Plaque is soft and removable with a toothbrush. But if it's not fully removed (and no toothbrush reaches every surface perfectly), it absorbs minerals from saliva and hardens into tartar (also called calculus) within 24–72 hours.

Tartar is mineralised — your toothbrush cannot remove it. Only a scaler can.

Scaling removes tartar from above and below the gumline using an ultrasonic scaler (which vibrates at high frequency to break up deposits) and hand instruments for precision work.

Polishing removes residual surface staining and the thin bacterial pellicle from enamel surfaces, leaving them smooth — which slows plaque reattachment.

What happens if tartar isn't removed

Tartar at and below the gumline is the direct cause of gum disease progression.

Stage 1: Gingivitis. Gums become inflamed in response to bacterial toxins in the tartar. They bleed when you brush. This is reversible with a good cleaning and improved home care.

Stage 2: Periodontitis. If gingivitis is untreated for months, the inflammation extends deeper. The bone supporting your teeth begins to recede. This is not reversible — lost bone doesn't grow back.

Stage 3: Advanced periodontitis. Significant bone loss. Teeth become loose. Extraction may be necessary.

Periodontitis also has systemic links. Research consistently connects it to higher risk of cardiovascular disease, poorly controlled diabetes, and — in pregnant women — preterm birth. Your oral health is not isolated from your overall health.

Why every 6 months specifically?

Research on tartar formation shows that most patients accumulate clinically significant buildup within 3–6 months of a professional clean. Six months is the evidence-based standard for patients with generally healthy gums.

Patients with active gum disease, diabetes, or a history of heavy tartar buildup are often placed on a 3-month recall instead.

The goal is to remove tartar before it has time to trigger inflammatory damage — not to react after the damage has started.

Does scaling damage enamel?

No — this is one of the most persistent myths. Ultrasonic scalers and hand instruments are designed specifically to remove deposits from enamel surfaces without scratching them. What you may feel after scaling — sensitivity, or teeth that seem "longer" — is actually your gums returning to their proper position after inflammation is reduced. The teeth aren't more exposed; the gums are healthier.

What to expect during the appointment

  1. Clinical assessment — your dentist checks pocket depths (the space between gum and tooth) and notes any areas of concern.
  2. Ultrasonic scaling — a water-cooled instrument removes the bulk of tartar. You'll hear a vibrating sound and feel mild pressure.
  3. Hand scaling — fine instruments reach into pockets and interproximal spaces.
  4. Polishing — a slightly gritty paste applied with a rotating cup removes surface stain.
  5. Fluoride (optional) — a fluoride application can reduce post-cleaning sensitivity.

The whole process takes 30–60 minutes. It should not be painful. If you have significant tartar or gum sensitivity, there may be mild discomfort — a good dentist will pause and adjust technique.

Karachi-specific considerations

In Karachi, a combination of high-sugar diets, hard water (which accelerates tartar formation), and inconsistent flossing habits means many patients accumulate tartar faster than average. Tea and coffee — both common daily drinks — also contribute to surface staining that polishing removes.

If you haven't had a clean in more than a year, your first appointment may take longer and may need to be split into two sessions for deep cases.

Prevention is almost always the cheaper path. The patients who say "I can't afford the cleaning" are the ones who end up spending ten times as much later.
